Activity Medical Form - At Your Own Risk Liability Form



Here we highlight another medical form.  This particular form envokes an at your own risk opt in.  Guess we can use this for mild or wild sports where there is a slim chance that an injury can occur.

Originally served up for yacht club members, the form can be modified to fit the needs of  little leagues, football teams, fitness activities, cheerleading, camps, or children’s play.
