Forms with a background image

Some of the html forms provided here at Download HTMLForms.com may include a background image for design purposes.  When installing these type forms, be sure to place the background picture file into the “images folder” found in the root or template directory.  If you don’t have an “image folder”, please create one.

The last step is to re-upload your modified files.

Making the Form Work - Contact.php

The contact.php file is what makes everything happen. Open contact.php and add your email in the EmailTo field. This file should be uploaded into the root directory.
